A HOME VISIT?
Home visits are reserved for those
patients who are too ill to attend the surgery and are done at the doctors’
discretion. Requests should be made before 10.00am if possible. Please
give our reception staff as much information as possible.

OBTAIN
A REPEAT PRESCRIPTION?
ORDERING
Repeat prescriptions can at present
be requested by post, in person, by telephone - after 10.00am, please - or by fax (01282 456924).
Whichever way you chose to order your prescription please ensure you give us full
details of your medication ie the name, strength and dosage used.
It is helpful when ordering to refer to the computer slip attached to your previous
prescription.
COLLECTING
Please allow 24 hours before collecting
your prescriptions. Certain chemists will collect prescriptions for you and in
some cases deliver the medication. Enquiries should be made to your local pharmacy.

REGISTER
WITH THE PRACTICE?
Those wishing to register with the
practice should collect a new patient pre-registration form from the surgery;
this should be completed and returned. This form will then be reviewed and, if
the pre-registration is acceptable, you will then be asked to either leave your
medical card - duly completed - or complete a Health Authority registration form.
You are then registered with the practice. You will be required to attend the
surgery to see the nurse for a new patient registration check. At this time you
should furnish the practice with as much information as possible about your past
and present medical history ie operations, ongoing problems, immunisations/vaccinations,
allergies etc.
All new patients are invited to make an appointment for a consultation to enable
the practice to obtain such information which may help the doctors and nurses
to offer specific patient-centred care.
PREFERRED
DOCTOR
When registering with the practice
patients do have the right to express a preference to see a particular doctor
and, whenever possible, the practice will accommodate these wishes. However, circumstances
such as emergency appointments, holidays etc sometimes prevent this. Patients
are assured however that they are still free to make appointments with either doctor.

Please notify reception of any changes
in your details including name, address and telephone number.

MAKE AN APPOINTMENT?
Appointments can be made by telephone or in person. Urgent cases will be seen
the same day but not always by your usual doctor.
If you cannot attend please cancel as soon as possible.

SPEAK TO A DOCTOR?
When you ring for an appointment you will be given the opportunity to have a telephone consultation with the doctor. If you agree, the doctor will ring you after the next surgery session (please ensure we have the correct telephone number on which to contact you). The receptionist will advise you of the approximate time you can expect this call.

OBTAIN TEST RESULTS?
Please avoid ringing first thing
in a morning. The best time to call is after 11.00am.

ENQUIRE ABOUT HOSPITAL FOLLOW-UP?
For routine enquiries after you have
been an in-patient or have been seen as an out-patient at the hospital, please
allow at least three weeks for any correspondence to be received by the practice
before making an appointment with the doctor.
